NICE will publish an appraisal of NRT and bupropion later this year. From April 2001 nurse prescribers have also been able to prescribe NRT and diltiazem.
The Tuberculosis Program of the U.S. Public Health Service studied INH prophylaxis in household contacts of new active pulmonary cases in the U.S. and Puerto Rico between 1957 and 1960. 8 The groups were similar in age and race distribution. Two thirds of all participants 20 years were randomized to receive INH or placebo for one year. Pulmonary or extrapulmonary tuberculosis developed in 78 of 12, 594 6.5 per 1, 000 ; placebo recipients versus 18 of 12, 439 1.5 per 1, 000 ; INH recipients. A similar household-based study in Japan failed to show a benefit of INH prophylaxis 9 although there was a trend toward decreased tuberculosis in the INH treated group 8 per 1, 142 ; compared to the control group 11 per 1, 096 ; . Poor compliance and the small sample size were the most likely reasons for the lack of significant effect. The Task Force concludes there is good evidence to support the role of INH chemoprophylaxis in household contacts of active cases of tuberculosis. There are many different ion channels that we know of, " said Dr. Devor. Some of these channels become more or less important as contributors to electrical impulses in hyperexcitable nerves. "One of the important questions is what changes occur in terms of the ion channels of nerve cells that render them hyperexcitable and therefore a cause of pain in neuropathy." Research shows that in dorsal root ganglion neurons capable of sustained firing, high-frequency subthreshold oscillatory potentials trigger spikes and spike trains. Neurons without oscillation potentials do not fire repetitively in response to sustained, physiological depolarizations. As a result, altering the expression of specific ion channels allows axotomy to facilitate oscillatory behavior. Researchers caution, however, that they are only now beginning to fully understand the extent of facilitating oscillatory behavior, and that there are many aspects of it that must still be explored. "I think the most important message to doctors treating neuropathic pain is that ion channels have a role in the treatment of all types of neuropathic pain, but I think drugs that are effective are probably not just those agents that take out the effect of ion channels. The ion channels story is only one piece of the puzzle to the whole transmission changes that you see in chronic neuropathic pain, " said Karin Petersen, MD, adjunct assistant professor of neurology at the University of California, San Francisco. Some recent research findings may have relevance to human neuropathic pain syndromes, including studies using experimental pain models and pharmacologic challenges to identify pain mechanisms, according to new data presented at the meeting.
