These neurochemical effects may result from indirect activation of DA neurons in the VTA 32 ; , as well as presynaptic modulation of DA synapses in the NAc 21 ; . Given the fact that glutamatergic afferents from the vSub also project to the dorsal PFC20, which in turn projects to both the VTA 33 ; and the NAc core 34 ; , either or both of these pathways could also contribute to the increase in DA efflux in the NAc evoked by vSub stimulation. Involvement of the vSubdorsal PFC pathway would link vSub stimulation-induced reinstatement of psychostimulant self-administration to the motor subcircuit which McFarland and Kalivas 18 ; hypothesize to be involved specifically in triggering and maintaining habitual patterns of drug-seeking behavior. Upper GI endoscopy and VCE were performed within 1-15 days of each other in all patients. Overall agreement between observers for the diagnosis of celiac disease by capsule endoscopy ranged between 79.2 and 94.4%; the values of kappa ranged between 0.56 and 0.87, denoting fair to excellent agreement. Conclusions: VCE endoscopy appears to be sensitive and specific for the detection of villous atrophy in patients with suspected CD.
